Important Note
The Magnus Health portal for 2022–23 is now open. We ask that you enter the health information required within your Magnus Health account for each child attending Greensboro Day School no later than August 12, 2022.
Health Forms & Magnus Health
GDS uses the Magnus Health SMR electronic health record (EHR) for all student medical information. We ask that you enter the health information required within your Magnus Health account for each child attending Greensboro Day School no later than August 12, 2022.
Please reserve 20 to 30 minutes per child, depending on your particular circumstances. For current families, we ask that you review and update your child’s health information. New families will need to enter their health information into the system, but the good news is that you will only have to repeat this work next year. Your data is securely stored, so going forward you will simply provide updates to the record as needed. You will receive weekly reminders as long as there are outstanding items in your account.
All sports physicals, medication authorization forms, and non-prescription medication forms must be uploaded directly into the portal after being signed by your child’s physician.

Log in to your myGDS account, click the Resources tab, and then click on the Magnus Health tile. The video below will walk you through the process.
Required Immunizations

Seventh Grade Students
All students entering seventh grade are required to have a Tdap (tetanus/ diphtheria/ pertussis) vaccine, if five years or more have passed since the last dose of tetanus/diphtheria toxoid, as well as one dose of meningococcal conjugate vaccine. Please consult your health care provider if your child has not yet received these vaccines.Each seventh grade student will be required to have an updated immunization record, with documentation of the required vaccines, uploaded into their Magnus Health Record by Thursday, September 15, 2022. Vaccine records may be uploaded to your child’s Magnus Health Record starting in mid-June, 2022. If the required immunization record has not been uploaded by September 15, 2022, your child will be unable to attend school until the record is received per North Carolina State Law 10A NCAC 41A.0401. -
Twelfth Grade Students
All students entering twelfth grade are required to have a booster dose of meningococcal conjugate vaccine. Please consult your health care provider if your child has not had two doses of this vaccine.Each twelfth grade student will be required to have an updated immunization record, with documentation of the required vaccine, uploaded into their Magnus Health Record by Thursday, September 15, 2022. Vaccine records may be uploaded to your child’s Magnus Health Record beginning in mid-June, 2022. If the required immunization record has not been uploaded by September 15, 2022, your child will be unable to attend school until the record is received per North Carolina State Law 10A NCAC 41A.0401.
