Greensboro Day School's experienced team of nursing professionals provides health care, first aid, and medical support during the school day. Their goal is to optimize a child's school performance utilizing their medical expertise and the information that parents provide regarding their child's health.
 
While your child is on the GDS campus, the health room is open for anything health related, from small bumps and bruises to significant health concerns.

The Health Room and the services it provides are an important part of a child's life at school, especially in the event of illness or injury. Our Health Room nurses get to know Greensboro Day School students and provide continuity of care as your child transitions from one grade to the next. 

The Health Room also provides vision and scoliosis screenings for students as needed.


Health Forms

Health Forms must be completed annually for each student enrolled at Greensboro Day School. 

All new students must provide an immunization record to your child's division prior to the beginning of the school year. Immunization records can be obtained from your pediatrician's office or your child's previous school.

Greensboro Day School uses Magnus Health for completion of Health Forms. 

For your children's healthcare provider appointments, please download the applicable forms for the provider's signature. This would include forms for the Sports Physical, Medication Authorization, Kindergarten Assessment, and care plans for chronic illnesses. You are able to upload these forms into your child's Magnus record. Please do not bring forms to the school. All forms must be uploaded into Magnus by August 12, 2022 and will be reviewed by the school nurse and athletic trainers.

Health Forms are available in your Magnus Health account and  include:
  • Health Information Form *Required for all students grades Bengal Tots (BT)-12. 
  • Early Childhood and Kindergarten health assessment form *Required for students grades BT-K.
  • Medication Administration form *Required for all students grades BT-12 and requires your pediatrician's signature.
  • An immunization record is required for all new students, all incoming Kindergarten students, 7th grade students, and 12th grade students.
  • Please complete allergy and anaphylaxis care, diabetes care, or seizure plan if applicable.
  • For students in grades 5-12 who will participate in athletics, a sports physical must be completed.

Chronic Health Concerns

Our Health Room staff have the skill set necessary to support families who have children with chronic care concerns such as allergies, asthma, diabetes, and seizures. Please complete and submit the necessary forms in your Magnus Health account.
The Health Center is conveniently located next to the cafeteria in the Kirby Administration Building. The Health Center is open from 7:45 a.m. to 3:30 p.m.Monday through Friday.
